I certify that the information given in this application is true and complete to the best of my knowledge. By signing this application, I authorize the store representative to investigate all statements made by me on this application form. I authorize the references listed on this application to give the store representative any and all information concerning my previous employment and release all parties from all liability for any damage that may result from
furnishing such information to you. I authorize Bob's Sub, Inc., herein know as "Company", its designated agent(s) to withdraw specimen(s) of my blood, urine, and hair for chemical analysis at any time. One purpose of this analysis is to determine or exclude the presence of alcohol, drugs, or othe substances. I authorize the release of the test results to Company. I understand that decisions concerning my employment and or prosecution will be made as a result of these tests. I understand that misstatements or omissions of information in connection with my application for employment can lead to rejection of my application or dismissal from employment whenever discovered. I also authorize the store representative to request references from my former employers. I understand that nothing contained in this employment application for in the granting of an interview is intended to create an employment contract between
the store and myself for either employment or for the providing of any benefits. No promises regarding employment have been made to me, and I understand that no such promise or guarantee is binding upon the store unless expressly made in a written contract of employment signed by me and an authorized
representative of the store. I understand and agree that if I should become employed by Bob s Sub, Inc., in consideration of my employment, I agree to abide by the rules and policies of the Company, including any changes made from time-to-time, and agree that my employment will be at-will, meaning that my employment and compensation can be terminated with or without cause, and with or without notices, at any time, at the option of either the Company, or myself.
Any and all statements/policies implemented by the Company may be changed in whole or part by the Company at any time, without advance notice and without explanation, reason, or consideration to any employee. Furthermore, the Company may change, in whole or part, any associate s compensations, assigned work hours, and/or assigned work location at anytime, without advance notice and without explanation, reason, or consideration to any employee.
